He was troubleshooting and adjusting some equipment, described only as a depositor. His finger got caught in a pinch point. He sustained the amputation of that finger. He was not hospitalized.

NarrativeAt 9:20 a.m. on November 27, 2017, Employee #1 was loading paper on a pallet in the robot palletizer area of a facility. during work, One of the robots dropped a 100 pound bag of flour on his back. As a result, Employee #1 sustained a broken back and injured right shoulder. He was hospitalized for his injuries.

NarrativeAt 12:45 p.m. on October 16, 2017, a 60 year old, male, truck driver was applying locks on a door of a refrigeration trailer. The employee was in the process of applying locks on the door of a refrigeration trailer. At the same time, a tractor/trailer that was across the parking lot began to roll away and ran into the employee and trailer, pinning the employee against the two trailers and causing fatal crushing injuries. The moving trailer did not have the brakes properly set which caused it to roll away from the building.
