NarrativeAt 9:30 a.m. on November 11, 2017, an employee was attempting to retrieve a pallet of lettuce from the second shelf of an industrial storage rack system using a forklift. The pallet broke in the process making it impossible to get the forks of the forklift all the way under the load and creating an unsafe condition. The employees did not want the load to drop so they used another powered industrial truck in an attempt to raise the bottom of the broken pallet. In the process of raising the broken pallet, the second forklifts forks also raised the cross beam that the pallet was resting on causing the rack system to start to come apart. In an attempt to get the cross beam back into place, the employee climbed on top of the forklift then stepped over to the rack system. The employee was holding onto the left cross beam of the rack system. It appears that the right cross beam pulled out of the vertical post it attaches to and struck the employees right hand. causing him to loose his grip and fall from the rack system. The employee flipped over on his way down causing him to strike the concrete floor head first and was killed.
On Monday, November 6th 2017 OSHA reps took a report of a serious accident in Albert City, IA at Ag Partners, Llc. A 49 year old Machinery maintenance occupation had been injured when the employee sustained burns from splash of anhydrous ammonia

NarrativeAt 8:45 a.m. on October 31, 2017, Employee #1 was performing maintenance on an anhydrous ammonia applicator. While performing the maintenance, the employee had to bleed the applicator down, and an unknown quantity of anhydrous ammonia that was trapped in the system was splashed onto the employee's right arm, chest and right side of face. Employee #1 sustained burns to the affected areas and was hospitalized.
OSHA investigators arrived in Rowena, SD on Friday, October 27th 2017 and began an accident investigation after learning of an incident at 96295 - Chs Inc Dba Chs Eastern Farmers which had occured on Thursday, October 26th 2017. A 82 year old employee had been killed when the employee fell down staircase and was killed

NarrativeOn October 24, 2017, an employee was working as a seasonal driver and was going down a set of stairs to the work floor. The employee fell from the third step from the bottom of the staircase and attempted to grab onto the stair railing with both hands. The employee turned his body 180 degree where he fell backwards to the wooden platform and then onto the cement floor. The employee was transported to the hospital and placed on life support. The employee suffered several fractured ribs and skull. The employee passed away on October 26, 2017.
