NarrativeAt 8:45 a.m. on October 31, 2017, Employee #1 was performing maintenance on an anhydrous ammonia applicator. While performing the maintenance, the employee had to bleed the applicator down, and an unknown quantity of anhydrous ammonia that was trapped in the system was splashed onto the employee's right arm, chest and right side of face. Employee #1 sustained burns to the affected areas and was hospitalized.

NarrativeOn October 24, 2017, an employee was working as a seasonal driver and was going down a set of stairs to the work floor. The employee fell from the third step from the bottom of the staircase and attempted to grab onto the stair railing with both hands. The employee turned his body 180 degree where he fell backwards to the wooden platform and then onto the cement floor. The employee was transported to the hospital and placed on life support. The employee suffered several fractured ribs and skull. The employee passed away on October 26, 2017.
OSHA investigators arrived in New Bethlehem, PA on Tuesday, September 19th 2017 and began an accident investigation after learning of an incident at Barrett Farm Supply Inc. which had occured on Tuesday, September 12th 2017. A 60 year old man working as a Laborer was injured on the job when the employee sustained fractured when caught in running machinery

NarrativeAt 2:00 p.m. on September 12, 2017, an employee was working for a firm that retailed or wholesaled gardening and farm supplies. A rope had fallen off a gear, and the employee was reattaching the rope to the gear. The rope ran from the first floor up to the second floor. The gear to which it was attached allowed a user to select a bin as the source of grain to be dispensed. When the employee reached behind some unguarded, operating machinery, he was pulled into that machinery. He sustained six broken ribs, a fractured sternum, and a cut to the left side of his head and face. He was hospitalized.
