NarrativeAt 4:30 a.m. on August 11, 2017, an employee was adjusting the tension of a pressure plate for an unguarded meat patty forming machine. NarrativeAt 4:45 p.m. on July 7, 2017, and employee was standing in the powered industrial truck (PIT) main aisle for a pallet build and talking with another PIT operator. The employee finished the conversation, turned, and took a step when the employee's right foot was run over by the wheel on a passing PIT (Crown stand-up rider truck model: RC 5545-40).
