NarrativeAt 7:00 p.m. on November 2, 2017, an employee was operating a motorized stand-up pallet jack in a warehouse setting. The employee was driving an aisle in a food distribution warehouse. The employee crashed into a set of shelving units that were adjacent to the aisle way. The employee was pinned between the pallet jack and the shelving units. The incident resulted in fatal injuries to the employee. Medical Examiners report determined that cause of incident was due to medical condition/event which caused the employee to lose control of pallet jack.

NarrativeAt 2:30 p.m. on October 30, 2017, Employee #1 was at the driver check-in area, scanning products. The operator of the electric power jack was attempting to pick up a pallet and struck the employee, who was working in the area. Employee #1 was caught between the wooden pallets and sustained fractures to his leg and ankle, requiring surgery. He was hospitalized for treatment.

NarrativeAt 11:30 a.m. on October 26, 2017, an employee was riding on the forklift's tines while elevated approximately 14 feet above the warehouse floor. The employee's coworker could not see him because of a piece of cardboard placed on the roll cage and the employee was facing backward so he could hold onto the back-rest for support. The employee was pinned between the fire sprinkler pipe and the forklift back-rest, suffering a fractured sternum and ribs along with a back sprain. The employee was hospitalized.
