NarrativeAt 8:15 a.m. on December 14, 2017, an employee was inspecting a newly delivered reach-lift to ensure safe operation. The employee was checking the mast and forks by raising and lowering them while checking for proper functioning. As the mast was being lowered, it tilted backwards and pinned the employee's head between the protective plastic barrier on the mast and steering control. The employee sustained a fractured spine.
OSHA investigators arrived in Spring, TX on Tuesday, November 14th 2017 and began an accident investigation at Certified Restaurant Service Center, Llc. A 72 year old Electrician had been killed when the employee was killed by arc flash

NarrativeAt 1:15 p.m. on November 10, 2017, three employees were to change out a 15 amp breaker from a Square D Power Style switchgear in a 480 volt, 2000 amp 3 phase system. The system was not de-energized. The employee was not wearing the proper PPE to protect himself from electrical shock or arc flash injuries. The employee shorted out the system when his driver, while connecting a conductor to one leg of the three phases, came into contact with the panel causing a short and subsequent arc flash. The employee was transferred to the hospital with critical wounds where he later died.

NarrativeAt 3:00 p.m. on June 20, 2017, an employee was unloading 10 lbs. boxes from a truck to a storage unit during a very hot day. The employee began to feel sick and was treated for syncope secondary to heat exhaustion. Hospitalization was not required.
