NarrativeAt 5:50 p.m. on July 22, 2017, an employee was placing a shipping/storage rack that was being moved by a forklift. The rack held approximately 19 sheets of glass measuring 5 feet by 10 feet by 25 inches. The employee was positioning the rack when several sheets of glass struck the employee as they fell. The employee died on July 24, 2017, from multiple injuries.

NarrativeAt 2:45 pm. on March 23, 2017, an employee was working from a 13 foot extension ladder placed on a sloped roof. The employee was making some roof flashing repairs when the ladder twisted and the employee fell to the ground. The employee was hospitalized with serious injuries for 5 1/2 weeks.
