NarrativeAt 4:00 p.m. on January 12, 2018, an employee was assisting with a lift of steel rods when the rigging failed and the load fell to the ground. The employee's leg was struck and fractured a bone by one or more of the steel rods, causing an unidentified injury. The employee was transported to the hospital, where he remained overnight for treatment.

NarrativeAt 9:00 a.m. on November 17, 2017, an employee was torch cutting the axle of a loader truck while a pressurized tire was still installed on the rim. The pressurized tire got into fire which caused the tire to blow out. The worker was struck and killed by a projectile of the outer lip of the rim on the upper body which tore his body vertically in half and was found 150 feet away from the location of the incident.

NarrativeAt 10:30 a.m. on November 16, 2017, a truck driver released the straps and got up on the flatbed. A forklift was standing by to unload the steel bar when the driver's feet became tangle in the straps causing him to fall off the truck on the passenger side. Simultaneously, the round stock had momentum and continued to roll and fell on employee killing him instantly.
