NarrativeAt 2:15 p.m. on November 28, 2017, Employee #1 was attempting to maneuver a forklift into position to lift a pallet. The employee backed the forklift into a ditch, flipping the forklift over, crushing and killing Employee #1.

NarrativeAt 2:45 p.m. on October 5, 2017, an employee was pushing a wheel barrow near the area of a traveling front end loader that was carrying a car by the forks. The front end loaders' forks were stuck through the cab of the car. The employee was struck by the car when the roof gave way and it fell from the front end loader. The employee suffered fractured legs and was hospitalized.

NarrativeAt approximately 3:30 p.m. on September 19, 2017, Employee #1 was working on an elevated vehicle. The vehicle was suspended on the forks of a forklift. While removing the vehicles cross member/motor mount to get to the rack and pinion. Employee #1 was struck in the head by a metal object, either the cross member/motor mount or the vehicle itself, when it fell. Employee #1 suffered blunt force head trauma and died.
