NarrativeAt 7:15 a.m. on October 31, 2017, Employee #1 was driving a forklift that he was loading onto a truck. When a coworker moved the truck, the forklift fell from the truck to the ground. The employee sustained back, and a left shoulder injury. The employee was hospitalized.
On Monday, August 14th 2017 OSHA reps took a report of a serious accident in San Antonio, TX which had occured previously at Manheim San Antonio. A 50 year old had been killed when the employee was struck and killed by a front end loader

NarrativeAt 10:30 a.m. on August 14, 2017, an employee was walking across the street from the mechanic shop to parking lot 355. While the employee was walking across the street, the John Deere 624K front end loader (PIN#: 1DW624KZCFF666864) was moving toward the employee at the same time. The front end loader was carrying a broken truck that restricted the front visibility of the driver. The employee did not realize that the front end loader was coming toward him. Also, there was no spotter at that time to alert the employee about the incoming front end loader. The fork on the front end loader struck the employee to the ground and the front end loader ran over the employee, causing a fatality accident.
On Thursday, February 23rd 2017 OSHA reps took a report of a serious accident in Brea, CA after learning of an incident at Harbor Truck Bodies which had occured on Monday, February 20th 2017. A 48 year old Machine operator had been injured when the employee's fingered are amputated in press brake

NarrativeAt approximately 11:30 p.m. on February 20, 2017, an employee was working in the fabrication area of the shop and was directly involved in the operation. The employee was operating a Wysong press brake, bending a small metal piece called a post, measuring approximately 3 in. by 15 in. The piece fell back into the machine and the employee disconnected his restraints to retrieve the piece. The press brake came down on his fingers. The employee sustained the amputation to the middle three fingers on his left hand for which he was hospitalized.
