NarrativeOn August 5, 2013, Employee #1 was flaring PVC piping. This process required placement of two flanges held together by two clamps prior to the flaring process. He held two flanges together with his hands rather than applying the clamps. As a result, his right middle finger was crushed between two flanges.
The accident investigation below was opened on Monday, April 4th 2011 by OSHA representitives in Chino, CA at Fisher Scientific Company Llc. A employee working as a Helper was injured on the job when the employee amputated finger between chain and sprocket

NarrativeAt approximately 2:00 p.m. on March 28, 2011, Employee #1 was performing preventative maintenance on section P130 of a conveyor (Hytrol Conveyor Company, Serial Number 451031). He turned off the power on the motor of the section P130, but he did not lock out the power switch. Employee #1 then sprayed a lubricant of its chain and sprocket and turned its power back on. As he wiped the extra lubricant from the chain with a rag, his right index finger was caught in a pinch point between the sprocket and chain, and was amputated. Employee #1 was transported to a medical center, where he underwent surgery and received post-operative care, after which he was released.
On Friday, June 26th 2009 OSHA reps took a report of a serious accident in Severn, MD which had occured previously at Diversified Distribution Systems. A Supervisors; distrib. had been injured when the employee receives electric shock from breaker switch

NarrativeUpon arrival at work on June 26, 2009, Employee #1 began turning on the lights f or a warehouse facility. When throwing the third breaker switches, the electric panel cover fell off, causing an arc flash. The panel was an ITE Panel operating at 225 volts, measuring 20 in. wide by 50 in. tall, and located 40 in. above a concrete floor. Employee #1 received bruises and minor electrical burns from the accident. An electrical company had been in the facility the week before servic ing the electric panel. It appeared that the panel must not have been replaced p roperly.
