NarrativeAt 10:00 a.m. on December 3, 2018, Employee #1 was operating a Diedrich D50 Drill Serial #423 to gather soil samples from a potential future construction site. The auger was brought out of the soil and while wearing leather gloves, Employee #1 used their hands to clean soil from the auger while it was still operating. Employee #1's hand became entangled in the auger and was drawn into/around the auger and against the machine. Employee #2, an assistant and not an operator, hit the e-stop and called 911. Employee #1 was pronounced deceased at the scene.

NarrativeOn December 19, 2017, an employee was supervising the excavation of a hole for a concrete foundation being dug out by a coworker. The employee was standing near the excavation and was struck by the excavator's back end when his coworker began to rotate the excavator's bucket. The employee was knocked into the 15 foot deep hole. The employee died later from his injuries.
