NarrativeAt 6:30 a.m. on November 30, 2018, an employee was gathering feed samples at a dairy to check on nutrition. The employee walked up to face of sheer wall of bulker of corn silage when a large section broke off landing on and engulfing him. The employee died of asphyxiation.

NarrativeAt 11:00 p.m. on September 14, 2017, an employee was working near Column B17 of a manufacturing plant and was cutting some aluminum material with a Rigid Dual Bevel miter saw. As the employee was cutting, the miter saw kicked back and hit his left pinky, causing a partial amputation. The employee was treated a San Jose Regional Medical Facility and was hospitalized.

The glider had a successful take off from the runway and was released by the tow vehicle. After take-off, the glider turned to starboard and then nose-dived into the ground, killing Employee #1.
