NarrativeAt 11:45 p.m. on November18, 2017, an employee who worked in a frozen vegetable processing plant was fixing the conveyor as it was not pushing empty totes to the side as it was supposed to. The employee removed the outside guard of the machine that covered the chain and sprocket drive system and then removed one of the two chains under the guard. The conveyor moved as the employee began removing the second chain and caught two of his left hands middle fingers in the chain and sprocket of the drive system. The employee was hospitalized and had two fingers amputated.
