NarrativeA horse was in a horse ring and was hooked to a carriage. The horse became spooked and started running, broke free of the reins, and attempted to exit the horse ring. An employee entered the horse ring to try and calm the horse. The horse struck the employee, who suffered a serious neck injury that required surgery, abrasions, contusions, and lacerations to the thighs/arms.
On Monday, June 13th 2016 OSHA reps took a report of a serious accident in Portageville, NY which had occured previously at Balloons Over Letchworth. A 34 year old man working as a was killed on the job when the employee was killed in fall from hot air balloon.

NarrativeAt approximately 9:00 p.m. on June 10, 2016, an employee was working as the gro und crew landing a hot air balloon. After the passengers exited the basket, th e balloon lifted off. The employee hung on to the basket as the balloon gained altitude. The employee lost his grip and fell approximately 100 feet. The emplo yee was killed as a result of multiple traumas from the fall.
