NarrativeAt approximately 9:00 p.m. on June 10, 2016, an employee was working as the gro und crew landing a hot air balloon. After the passengers exited the basket, th e balloon lifted off. The employee hung on to the basket as the balloon gained altitude. The employee lost his grip and fell approximately 100 feet. The emplo yee was killed as a result of multiple traumas from the fall.

NarrativeAt approximately 4:40 p.m. on July 1, 2011, Employee #1 was in the process of shutting down a De Immigrant Windmill with a coworker. Employee #1 was installing wooden chocks in the top most wooden wheel in the cap of the windmill. He fell approximately 13 ft through an unguarded floor opening onto the mid-level wooden spur wheel. He was hospitalized and later died.

NarrativeAt about 9:56 p.m. on July 28, 2010, Employee #1 and a coworker, regular employe es of the employer, were filling six 30-gallon propane gas cylinders that were i n a balloon ride basket inside an enclosed trailer, which had license number 4KL 4897. Employee #1 was filling the cylinders inside the trailer, and the coworker , toward the rear end of the trailer, was helping him. As Employee #1 was fillin g the cylinders, a flash fire occurred from an unknown source of ignition. Emplo yee #1 sustained second-degree flash burns on fifteen percent of his body. He re quired hospitalization for forty hours. The coworker did not receive any serious injury. The employer had total of four employees and was nonunionized.
