NarrativeA horse was in a horse ring and was hooked to a carriage. The horse became spooked and started running, broke free of the reins, and attempted to exit the horse ring. An employee entered the horse ring to try and calm the horse. The horse struck the employee, who suffered a serious neck injury that required surgery, abrasions, contusions, and lacerations to the thighs/arms.

NarrativeAt 8:30 a. m. on December 16, 2014, Employee #1, a railroad worker, climbed into the top third level (C-deck) of a Tri-level Auto-rack railcar #700G96, to check and verify the tire securement of a yellow Chevrolet Camaro. He checked the chock and straps of the car, to see if they were removed. After the employee went around to the front of the car, he moved backwards and fell from the C-deck through a 3 foot 6.5 inch gap opening between two bridge plates at an elevation of more than 13 feet above the rail road tracks. Employee #1 fell and struck the tracks and sustained head trauma and multiple body fractures, killing him.
