NarrativeAt 8:30 a.m. on March 19, 2019, an employee was using a flat-bed car carrier to retrieve a truck that had broken down. After the disabled truck had been loaded onto the car carrier and the employee worked to secure it, the truck rolled forward. The employee became pinned between the truck and the flat-bed car carrier, killing him. The incident investigation determined that the truck had no brakes.

NarrativeAt 8:20 a.m. on February 8, 2019, an employee was located under a semi-truck, assessing the extent of a broken drive shaft, when a coworker began pushing the vehicle into a service garage. The employee was run over and crushed by the truck's wheels, killing him.

NarrativeAt 2:15 p.m. on February 5, 2019, an employee was guiding a forklift operator to load 10 ton counterweights onto a tractor trailer. During the lifting operation, the employee placed his head between two stacks of counterweights and signaled to the forklift operator to lift the counterweight. As the load was raised, the counterweights shifted, crushing the employee's head. The employee was killed as a result of his injury.
