NarrativeAt 12:00 p.m. on September 6, 2017, Employee #1 was fueling a chainsaw near a Hitachi 250 tracked log loader, during helicopter logging activities. The employee's hand was run over by the tracked loader and became caught between the track and saw. Employee #1 suffered partial amputation to the third and fourth digits of his left hand. He was hospitalized for his injuries.

NarrativeAt 3:00 p.m. on September 1, 2017, an employee was making deliveries and notified the employer that he was not feeling well. The employer directed the employee to stop work, seek shade, and cool off. The employer drove the employee, provided first aid, and offered to take the employee to the hospital. The employee declined, but was later taken by his wife to the hospital that night for dehydration.
