NarrativeAt 12:00 p.m. on September 10, 2019, an Employee was plastering a building in a commercial construction project and while climbing the fifth level of a scaffold ladder, the Employee fell, then struck a metal fence, and landed on the ground. The Employee sustained a ruptured aorta and severed spinal injury and upon emergency transport, coded three times and was then pronounced dead at the hospital.

NarrativeAt 8:00 a.m. on July 29, 2019, Employee #1, employed by a construction company, was part of a work crew at a multiemployer residential construction project. The crew, upon arrival at the job site, was preparing for the workday. Shortly after arrival, Employee #1 was found face down on the ground near a scaffold. Coworkers reported a smell of alcohol on his breath. Employee #1 declined medical attention and was taken home by the crew's foreman. Employee #1 later was found dead in front of his residence. The autopsy revealed multiple fractured vertebrae at the thoracic level, traumatic injury (transection) to the spinal cord, multiple rib fractures, and contusions and abrasions at the thoracic level. The medical examiner was unable to determine the cause of the injuries.
The accident investigation below was opened on Monday, July 29th 2019 by OSHA representitives in Staten Island, NY which had occured previously at Leone Concrete Inc.. A 57 year old had been killed when the

NarrativeAt 11:00 a.m. on July 25, 2019, Employee #1, employed by a concrete company, was engaged in exterior masonry work at the construction site for a commercial building. As he dismantled the scaffold he had been working from, he fell backward off the end of the scaffold's work platform, a fall height of 12 feet. Employee #1 sustained a traumatic brain injury/brain hemorrhage and was killed.
