NarrativeAt 1:00 p.m. on September 1, 2017, Employee #1, employed by a countertop manufacturer, was operating a forklift to move a dumpster to the curb. The dumpster fell off the forks. Employee #1 was pinned between the forklift and a company van. Emergency services were called, and Employee #1 was transported to the hospital. He was admitted and treated for an injury to his right leg.

NarrativeAt approximately 3:30 p.m. on June 12, 2017, Employee #1 was cutting and removing damaged from porcelain tile on the ground. The tiles had been offloaded in an A frame crate from a flatbed truck, onto the sidewalk adjacent to the site. Each porcelain tile weighed approximately 200 pounds and had dimensions of 5 ft by 10 ft. The remaining undamaged tiles were placed on a rack behind him. As he worked, the rack tipped over and struck him. Employee #1 suffered crushing injuries and fractures in both hips. Employee #1 was transported to a medical center, where he received treatment and was then hospitalized.
