NarrativeOn December 14, 2018, an employee was sitting inside a company van while on lunch break and was discovered slumped over the steering wheel, unresponsive and without a pulse. The employee was transported to a hospital and pronounced dead from carbon monoxide poisoning.

NarrativeAt 7:00 p.m. on November 23, 2018, Employee #1 was troubleshooting an elevator. In the elevator room on the roof of the building, he bypassed the door safety system with a jumper wire and did not "gain control of the elevator" by putting the elevator in the inspection mode. Employee #1, went back down to the first floor lobby of the building to the elevator location and was inside the elevator with the door opened when the elevator suddenly dropped. Employee #1 tried to climb out of the reduced opening to the lobby and while climbing through the opening to the lobby floor, the elevator car dropped a second time crushing the employee's torso between the top of the elevator car and the lobby floor.

NarrativeAt approximately 2:30 p.m. on November 12, 2018, an employee, a laborer, was dismantling a portion of the C7 Pastuerizer line in beverage facility. The employee was using a Stihl TS 420 Cut Off Saw (serial number is unknown at this time) equipped with a 14 inch Diablo metal cut off abrasive wheel to separate a portion of the stainless steel frame that had the company's nameplate welded onto it. While making one of the cuts, the cut off saw kicked back, striking him in the left shoulder and neck. A coworker, a manager, was approaching the employee when the accident occurred and assisted the employee down the steps where he then staggered and fell. The employee made it approximately 25 feet from the point of the accident before collapsing. Another employee, the job superintendent, called 911 and was directed on how to assist the employee as well as where to meet the ambulance. When EMS arrived on scene, they pronounced employee as dead on arrival.
