NarrativeAt 10:00 a.m. on January 14, 2019, an employee was completing some repairs to a metal building and fell through an existing skylight. The employee fell approximately 16 feet and landed on his head, fracturing his skull. The employee was hospitalized for 8 days before being removed from life support and died once removed.

NarrativeAt 2:32 p.m. on January 12, 2019, an employee had welded a steel post to the uppermost beam on a steel structure. The employee was wearing an Ultra-Safe brand safety retractable lanyard and harness. During the fall, the fall arrest system failed and the employee was killed. The employee fell 48 feet to the ground.

NarrativeAt 10:20 a.m. on December 17, 2018, an employee was retrieving cement blocks from a crane basket to place them in two locations where a wall was being built. The employee was walking on some plywood that was not supported underneath and the employee fell approximately 40 feet to a set of rebar that impaled him, killing him.
