NarrativeAt 2:38 on October 2, 2018, an employee was struck in the chest by a accidental gun discharge. A coworker had .32 caliber semi-automatic gun that he had pointed in the direction of the employee when the gun discharged and struck the employee in his chest, killing him.

NarrativeAt 12:15 p.m. on December 20, 2017, an employee was assisting a coworker with unloading a truck that was full of granite slabs. As the coworker operated a forklift to transfer the unloaded slabs of granite from the back of the truck to a v-rack, the employee took a break and was sitting on the ground near the rack. When the employee pulled himself up, grabbing a hold of approximately 5 to 6 granite slabs that were on the rack, the rack overturned. The granite slabs and rack fell on the employee, fracturing his right fibula. The employee was taken to a nearby hospital where he was admitted and underwent ACL reconstruction.

NarrativeAt 9:30 a.m. on December 5, 2017, Employee #1 was setting up the forks on the forklift and manually moving it by kicking the fork to where it need to be. Employee #1 was holding on to the load backrest (moved left and right) of the fork lift and the forklift operator engaged the lever causing the load backrest to move. Employee #1's hand was sheared between the backrest and another vertical member of the forklift. Employee #1 fingers were amputated.
