NarrativeAt 9:30 a.m. on December 5, 2017, Employee #1 was setting up the forks on the forklift and manually moving it by kicking the fork to where it need to be. Employee #1 was holding on to the load backrest (moved left and right) of the fork lift and the forklift operator engaged the lever causing the load backrest to move. Employee #1's hand was sheared between the backrest and another vertical member of the forklift. Employee #1 fingers were amputated.
On Monday, November 13th 2017 OSHA reps took a report of a serious accident in Columbus, OH after learning of an incident at Thrift Stores Of Ohio,Inc. which had occured on Thursday, November 9th 2017. A 51 year old Laborer had been killed when the employee was killed when crushed under forklift

NarrativeAt 3:30 p.m. on November 9, 2017, Employee #1, a 51 year old male, was loading a truck at a loading dock using a forklift. The truck pulled out from the dock causing the forklift to fall out of the back of the truck. The forklift fell on top of the employee causing crushing injuries that resulted in his death.
