NarrativeAt 3:27 a.m. on May 21, 2019, an employee at a steel manufacturer was troubleshooting a fault on the galvanizing line. The employee was in the exit pit of the line and placed a barrel between the belt and the tension reel. The employee was climbing on the barrel when the belt wrapper engaged and the employee became pinned between the belt wrapper and the reel. The supervisor was at the scene, and the equipment was not locked or tagged out.

NarrativeAt 12:30 a.m. on February 9, 2019, an employee was standing on the ledge of a pickle tank that contained 160 degree Fahrenheit sulfuric acid. As the employee reached down with a plastic syringe to collect a solution sample, he fell into the tank. The employee suffered chemical exposure and thermal burns, killing him.

NarrativeAt 10:00 a.m. on December 28, 2018, and employee and coworker were using an overhead crane to move a piece of equipment that weighed approximately 6-10 tons in order to perform maintenance. The employee was operating the crane with a remote control and was standing next to some large sand molds. The employee was struck by the crane as it moved and was killed.
