NarrativeAt 3:00 p.m. on June 29, 2017, Employee #1 was working from a ten foot A-Frame ladder to trim a tree with a chainsaw using. As the employee cut a branch, it fell to the ground, and rolled towards the ladder. The branch struck the ladder, causing the employee to lose his balance and fall from the ladder and drop the chainsaw. He landed on the saw, sustaining a laceration to the right side of his lower back. The employee was hospitalized and released 3 days later.

NarrativeAt 1:30 p.m. on June 20, 2017, an employee was pulling wires in an attic and standing on a weight bearing platform. The employee was holding onto a structure in the attic and lost his balance before falling through the ceiling and landing on a concrete wall. The employee suffered ten broken ribs and was hospitalized for approximately 5 days.

NarrativeAt approximately 9:15 a.m. on February 22, 2017, Employee #1 was operating a foot pedal on a snap press, GZN-2 semi-automatic button machine, while her finger was still in the area between the die and punch. The press caught her fingertip in between the die and punch, crushing her fingertip, resulting in a fingertip amputation and bone loss. The employee was not hospitalized.
