NarrativeAt 7:15 a.m on October 9, 2017, an employee was turning a ball valve handle and sulfuric acid leaked from a gasket. The employee suffered a chemical burn and was hospitalized.
The accident investigation below was opened on Tuesday, October 10th 2017 by OSHA representitives in South Jordan, UT which had occured previously at Rio Tinto Kennecott Copper. A 64 year old Miscellaneous plant had been killed when the employee was killed from sulfur dioxide inhalation

NarrativeAt 2:00 a.m. on October 8, 2017, an employee was clearing slag at the head end of the FS Boiler. A booster blower "tripped" causing the draft pressure to go from a negative to a positive pressure that resulted in a release of sulfur dioxide. The employee inhaled sulfur dioxide damaging lung tissues. The employee was killed.

NarrativeAt 9:20 a.m. on September 28, 2017, Employee #1 was cutting seven foot catalytic converters into three pieces with a band saw. The employee got his glove caught in the blade of the saw, and sustained amputations of his middle and index fingers of the left hand. Employee #1 was hospitalized for his injuries.
