NarrativeAt 5:30 a.m. on November 27, 2017, Employee #1 was preparing to run a glue machine. There was a dried piece of glue impeding the roller movement. The employee reached through an opening in the roller guard to retrieve and remove the quarter size piece of dried glue. Upon removal of the dried glue piece, the rollers engaged, and his finger was pulled into the pinch point of the rollers. The employee tripped the safety bar, which reversed the roller direction and released his finger. Employee #1 suffered an amputated right index fingertip.

NarrativeAt 5:30 a.m. on October 21, 2017, an employee was cleaning an index sensor on the A40 bracket welder which did not clear the machine fault. The employee attempted to clean the actuator and the machine cycled itself, causing the employee to suffer an amputation. However, the initial report did not identify what body part had been amputated. Hospitalization was not required.
