NarrativeAt 5:30 a.m. on October 21, 2017, an employee was cleaning an index sensor on the A40 bracket welder which did not clear the machine fault. The employee attempted to clean the actuator and the machine cycled itself, causing the employee to suffer an amputation. However, the initial report did not identify what body part had been amputated. Hospitalization was not required.

NarrativeAt 10:30 p.m. on August 16, 2017, an employee was operating an upward press brake and the employee dropped a metal plate onto the floor before putting his hand into the press. The employee depressed the foot pedal which activated the press and amputated his right hand's ring finger and pinky. Hospitalization was not required.

NarrativeAt 9:20 a.m. on July 21, 2017, an employee was working for a firm that restored wooden components and accessories such as: cabinets, shelving, and lockers; on yachts. He was using a table saw to rip pieces of wood. The table saw lacked a guard. He was using a push stick to push a piece of wood through the table saw. The stick slipped or broke, and his left hand was caught by the blade. He sustained lacerations to four fingers on his left hand. There was bone loss in three fingers. He was hospitalized.
