NarrativeAt 1:30 p.m. on October 19, 2017, an employee was adjusting machinery and was struck by a rotating blade of an Edgebander Unit, suffering a severe laceration. The employee required an amputation approximately one week later. Hospitalization was not required.
OSHA investigators arrived in Santa Clara, CA on Tuesday, September 5th 2017 and began an accident investigation after learning of an incident at Complete Millwork Services, Inc. which had occured on Wednesday, August 23rd 2017. NarrativeAt 8:45 a.m. on August 23, 2017, an employee was cutting a wooden slat for a door panel using a Dewalt DWE 7491 table saw. The employee removed the table saw guard and was making a fine cut when his left hand came into contact with the saw blade and lacerated all of his fingers. The employee was hospitalized.

NarrativeAn employee was cutting a flat piece of cork stock (1/8" thick from a 12"X12" section) to be used as trimming on furniture. He was having difficulty cutting the cork material because it would not slide on the table saw surface due to the cork material adhering to the table. He proceeded to apply more force, and as he attempted to push the stock into the blade, the material inadvertently folded, causing his left hand to move into the rotating blade. His left-hand pinky finger was amputated, and his ring finger was lacerated.
