NarrativeAt 5:30 a.m. on November 27, 2017, Employee #1 was preparing to run a glue machine. There was a dried piece of glue impeding the roller movement. The employee reached through an opening in the roller guard to retrieve and remove the quarter size piece of dried glue. Upon removal of the dried glue piece, the rollers engaged, and his finger was pulled into the pinch point of the rollers. The employee tripped the safety bar, which reversed the roller direction and released his finger. Employee #1 suffered an amputated right index fingertip.

NarrativeAt 2:00 p.m. on July 31, 2017, Employee #1 was using a rag to clean the components of a Brandt KD69 edgebander machine, Serial number 0-261-03-5880. The employee opened the hood located on the back of the machine and, unaware that the machine was on, proceeded with cleaning. Employee #1's left index finger contacted a rotating part, resulting in a severe laceration to the end of the his finger. The employee was hospitalized for surgery. It was determined that the machine's guard door interlock was not operable, and energy control procedures were not developed and applied for the employees who clean and service the equipment.
