NarrativeAt 11:15 p.m. on October 11, 2017, an employee was flushing out a heat exchanger. The employee sustained second and third degree burns to the back, arms, chest, stomach, and the left side of the neck when contacting hot water. The employee required skin graphs treatments.

NarrativeAt 6:30 a.m. on August 4, 2017, Employee #1 was operating a pump. The pump became clogged and the employee tried to unclog it. As he disconnected the feed hose from the pump, the employee stuck his right index finger into the pump to try to dislodge whatever was causing the backup. Employee #1's finger became caught in the pump and was amputated.

NarrativeAt 11:30 a.m. on May 7, 2017, an employee was checking the loosen wire connections on an electrical panel. After checking wire connection on the electrical panel, the employee was putting it back together when his screw-driver came into contact with the energized electrical box which created an arc flash and injured his face and arm. The employee suffered second and third burns to face and arm for which he was hospitalized..
