NarrativeAt 9:15 a.m. on January 1, 2018, an employee was walking in the Seafood Processing Room. There was ammonia in the air from an ammonia leak the night before. The employee stopped, was staring, put his hands into the air, and then collapsed onto the floor. While on the floor, the employee was shaking as if having a seizure and then stopped breathing. According to the Certificate of Death, the immediate cause of death was anoxic brain injury including acute liver failure, acute renal failure, and hypernatremia. The manner of death was recorded as natural causes.

NarrativeAt 4:15 p.m. on August 23, 2017, an employee was operating a Foretress Metal Detector which is used to detect any metal in the salsa that was being prepared. The employee reached into a bypass valve with her hand when it became stuck and the valve closed, pinching and lacerating the employees left ring and middle fingers. Hospitalization was not required.

NarrativeAt 3:30 p.m. on August 3, 2017, Employee #1 was operating a drum winch, utilizing ship rope to pull can stacks from Retort Number 4. The rope became bound on the left side of the drum. The employee attempted to move the rope without shutting the machine down. Employee #1's hand became caught in the pinch point between the rope and gear teeth. The employee sustained multiple finger amputations, and was hospitalized. The fingers were later reattached in surgery.
