NarrativeAt 12:15 p.m. on December 6, 2017, an employee was observing the operations of the company's #4 Hayssen Bagging Machine, Model 12-16SF, Serial # 086013, because it was not operating properly and the bags were jamming. While observing, the machine jammed and the employee began pulling down bags toward the machines jaws. The employee caught his left hand ring finger in the machines jaws when it cycled and amputated his fingertip. Hospitalization was not required.

NarrativeAt 2:00 a.m. on November 1, 2017, an employee was removing pecans from between the guard on a power transmission motor and a metal shield beside the conveyor belt. While using his left hand with his fingers straight out and pointing downward, the employee was pushing pecans out from between the power transmission motor guard and a metal shield on the side of the conveyor when the tip of his middle finger went into an opening on the side of the guard that covered the chain drive and sprocket(s) of the power transmission that powered the conveyor belt. The flesh tip of the employees middle finger on his left hand was amputated. The "torque" sprocket that was behind a guard that had a small opening so that the guard could fit over the shaft.

NarrativeAt 2:30 on October 28, 2017, an employee was cleaning flour from the rim near an actuating valve which was not locked out. The employee amputated the tip of his left index finger when the valve closed on his finger. The employee was hospitalized.
