NarrativeAt 2:00 a.m. on November 1, 2017, an employee was removing pecans from between the guard on a power transmission motor and a metal shield beside the conveyor belt. While using his left hand with his fingers straight out and pointing downward, the employee was pushing pecans out from between the power transmission motor guard and a metal shield on the side of the conveyor when the tip of his middle finger went into an opening on the side of the guard that covered the chain drive and sprocket(s) of the power transmission that powered the conveyor belt. The flesh tip of the employees middle finger on his left hand was amputated. The "torque" sprocket that was behind a guard that had a small opening so that the guard could fit over the shaft.

NarrativeAt 1:30 p.m. on October 20, 2017, an employee was removing a jam from a Urschel 2500 Trans-Slicer food processor when two of his fingertips were caught in its blades. The employee's right hand middle and ring fingertips were amputated. Hospitalization was not required.

NarrativeOn or about September 6, 2016, at 5:20 p.m., a temporary employee was attempting to remove a piece of plastic from a running machine when the machine caught and partially amputated the employee's right index finger.
