NarrativeAt 1:15 a.m. on April 14, 2019, an employee was near a roof top exhaust fan when he fell from the roof. The employee sustained head injuries as well as other body fractures and was killed when he struck the ground.

NarrativeAt 10:30 a.m. on December 5, 2017, an employee was rip cutting maple wood boards using a Tannerwitz table saw Model #J250, Serial #8078. The employee was struck in the abdomen when one of the boards kicked back while being cut. The employee was taken to the hospital where he was underwent surgery for blunt force trauma to his abdomen. The employee died a few hours later.

NarrativeAt 9:15 a.m. on October 25, 2017, an employee was working for a manufacturer of spices and extracts. The facility had a glass fermentation vessel used for distilling a dairy byproduct. He was cleaning this chemical reaction vessel and he was not paying attention to fluid levels. A safety valve opened and released hot cleaning liquid onto him. He sustained thermal burns and possibly some chemical irritation. He was hospitalized.
