NarrativeAt 10:00 a.m. on November 6, 2017, an employee was installing a die in a hydraulic press. When the employee inadvertently actuated the machine's foot switch, the press cycled and the die contacted his right hand. The tip of the employee's right ring finger was amputated.

NarrativeAt 11:45 p.m. on October 26, 2017, an employee was setting up the wire feed roller, jogged the machine, and reached the hazardous pinch area prior to the feed line stopping. The employee's hand was pulled into the pinch point and amputated several fingertips. Hospitalization was not required.
