NarrativeAt 9:00 a.m. on December 15, 2017, an employee was cleaning the excess oil from the bottom of a tanker rail car following a previous application of oil to prevent corrosion. The employee collapsed due to a 98% atmosphere of nitrogen being present inside the tank car. The employee was killed due to asphyxiation.

NarrativeAt 2:45 p.m. on November 17, 2017, an employee was welding a 16 inch pipe flange on to a reducing steel pipe elbow. The employee had made a first pass weld on the flange and repositioned himself to make a second pass weld. The elbow was on three pipe stands and fell as the employee moved, crushing his leg. The employee was hospitalized.

NarrativeAt 1:00 p.m. on August 29, 2017, Employee#1 was delivering water tanks with gravel bins to a new residential site in the mountains. As he unloaded a gravel bin and a tank, he placed the empty gravel bin onto the trailer without securing it to the trailer. When he tried to move the second gravel bin, it slid and struck Employee #1, knocking him to the ground. The employee sustained a fractured hand and leg, and was hospitalized for his injuries.
