NarrativeAt 9:00 a.m. on December 15, 2017, an employee was cleaning the excess oil from the bottom of a tanker rail car following a previous application of oil to prevent corrosion. The employee collapsed due to a 98% atmosphere of nitrogen being present inside the tank car. The employee was killed due to asphyxiation.

NarrativeAt 2:15 p.m. on December 6, 2017, an employee was working inside a permit-required confined space while performing some welding activities. The employee walked through a ventilation duct to an adjacent ash tower where some dried ash broke free from the side of the tower and fell down on top of the employee.
