NarrativeAt 7:45 a.m. on December 7, 2018, an employee was preparing to do welding on the rollers in the quenching water tank operations area. The equipment was not locked out, and the employee tripped the motion sensors that allowed for movement of press near wash/drying area, while employee was in a working position. He was crushed by the downward movement of press and was killed from internal bleeding.
OSHA investigators arrived in Baton Rouge, LA on Wednesday, October 17th 2018 and began an accident investigation which had occured previously at Performance Contractors, Inc. A 27 year old Pipelayer had been killed when the employee fell 80 feet and was killed

NarrativeAt 7:25 a.m. on October 16, 2018, an employee went to the second level of an exchanger and fell 80 feet to a platform that was approximately 20 feet from the ground. The employee was killed.

NarrativeAt 9:30 a.m. on September 21, 2018, five employees from two different companies were loading large machine shop equipment onto a flat bed truck during the liquidation of one of the companies. While loading a large tube bender on the truck using a forklift, an unsecured mechanical press brake, previously loaded onto the truck, listed and fell from the truck bed and landed onto one of the employees. The employee sustained fatal blunt force trauma / crushing injuries with the cause of death listed as exsanguination.
