NarrativeAt 8:00 a.m. on April 26, 2019, an employee was spotting a forklift. The 720 kilogram load on the forklift fell and crushed the employee, killing him.
OSHA investigators arrived in Visalia, CA on Tuesday, September 25th 2018 and began an accident investigation after learning of an incident at Milky Way Farms which had occured on Monday, September 24th 2018. NarrativeAt 4:30 p.m. on September 24, 2018, Employee #1 was standing on the peak of the sheet metal roof of the north protective cattle structure. He was removing screws from sheet metal and light panels while Coworker #1 and Coworker #2 were working from a scissor lift below. While standing on the two purlins that run the length of the structure, Employee #1 fell through one of the light panels to the cement ground below, approximately 24 feet, and was killed.

NarrativeAt 7:45 a.m. on April 11, 2018, an employee was moving dairy cows to feed them and/ or clean out the floor area. During work, the employee was struck or pinned by one of the animals that was moving rapidly in a penned in area. The employee was killed as a result.
