NarrativeAt 4:30 p.m. on September 24, 2018, Employee #1 was standing on the peak of the sheet metal roof of the north protective cattle structure. He was removing screws from sheet metal and light panels while Coworker #1 and Coworker #2 were working from a scissor lift below. While standing on the two purlins that run the length of the structure, Employee #1 fell through one of the light panels to the cement ground below, approximately 24 feet, and was killed.
On Wednesday, October 10th 2018 OSHA reps took a report of a serious accident in Saline, MI after learning of an incident at D & M Marion Inc which had occured on Wednesday, April 11th 2018. A 64 year old had been killed when the employee was struck or pinned by dairy cow and was killed

NarrativeAt 7:45 a.m. on April 11, 2018, an employee was moving dairy cows to feed them and/ or clean out the floor area. During work, the employee was struck or pinned by one of the animals that was moving rapidly in a penned in area. The employee was killed as a result.
On Monday, December 11th 2017 OSHA reps took a report of a serious accident in Haviland, OH at Greentop Acres, Llc. A 31 year old man working as a Laborer was killed on the job when the employee fell from roof and was killed from related injuries

NarrativeAt 4:45 p.m. on December 9, 2017, an employee was installing a rigid foam insulation board on the top cupola that was located on the dairy parlor's roof. The employee accessed the metal roof using a bucket truck from the west side of the building and the employee fell approximately 22 feet to the ground on the east side of the building. The employee was transported to Parkview Hospital where he died from fall-related injuries.
