NarrativeAt 12:15 a.m. on October 8, 2017, Employee #1 was pulling a hemodialysis machine into an elevator when its wheels became caught in the gap between the elevator car and the hallway floor. As the employee worked to free the wheel from the gap, the machine fell into the elevator and onto the employee. The employee sustained a fractured spine at the L1 lumbar region.
