NarrativeAt 4:32 p.m. on October 23, 2017, an employee had completed a methyl bromide fumigation for an offsite office and had transferred some methyl bromide from a larger cylinder to a smaller one for the job. The employee transferred the chemicals while in the garage portion of the fumigation company's building and completed the off-site fumigation. The employee returned to the company's building and continued other work. The employee was later found incoherent by a coworker and police. The employee was transported to a local hospital where he later died. The coworker discovered that the larger methyl bromide cylinder had been leaking in the garage area.

NarrativeAt 1:00 p.m. on October 10, 2017, an employee was walking on a roof to inspect for rat entry points, when he fell through a skylight. The employee struck a warehouse floor below and sustained several broken bones, which he was hospitalized and received treatment.

NarrativeAt 5:00 p.m. on October 2, 2017, an employee and coworker were relocating a bee hive from a customer's private residence backyard tree. The employer is a local company who relocates bee's. The employee climbed a ladder while a coworker held the ladder in place. The employee secured the ladder to the tree and climbed down and went to get fall protection. Upon returning, the coworker was in the tree. The employee climbed the tree using the tree trunk. After climbing five feet into the tree, the employee reached out to a tree branch about five feet higher. The employee grabbed the branch and attempted to pull himself up. The branch broke and the employee fell to the ground. The employee cannot remember the events proceeding the branch braking. The employee sustained a fractured right arm above the wrist. The employee was treated at Scripps Mercy Hospital and was in the hospital for more than 24 hours. OSHA was notified on September 18, 2017 at 3:00 p.m. The employee was a lead wrangler and worked for the employer for two years. The employee who fell was in charge of the project.
