NarrativeAt 5:15 a.m. on October 17, 2017, an employee was placing tires on racking system using an aerial lift. The employee fell more than 20 feed and landed on the concrete floor. The employee sustained fractures to his shoulder, elbow, and knee. The employee also sustained a lip laceration and the loss of three teeth. Fall protection was not being used.

NarrativeAt 10:00 a.m. on July 13, 2017, Employee #1 was inflating a split rim tire. The locking ring component of the split rim exploded and popped out, hitting the employee on the hand and on his face. Employee #1 sustained a broken hand and left facial trauma, resulting in the loss of an eye. The employee was hospitalized for his injuries.
