NarrativeAt 2:00 p.m. on December 13, 2018, an employee was working with a coworker and they were attempting to pull five stacked casting skids along a holding rail using pneumatic winches in the blast area of a foundry. The employee had attached an open hook from the north Ingersoil Rand Air Motor, Model EU, Serial Number RCA91059, and had pulled the slack out of the cable which unbalanced the stack. The falling casting skids toppled over on top of the employee, killing him.

NarrativeAt 3:15 a.m. on November 14, 2018, an employee was spotting for the steel side sheet and fell onto the roller bed. The employee's head and neck hit the roller and it appears that the roller grabbed her jacket. She sustained internal head trauma and later died at a local hospital.

NarrativeAt 8:45 a.m. on October 27, 2017, an employee was installing running boards on the top of a railcar and disconnected his fall protection to climb down the train car to turn on the air source. The employee pulled on one of the running boards on top of the car and fell off the ladder while climbing back up. The employee suffered head and facial injuries and was hospitalized.
