NarrativeAt 7:00 p.m. on September 15, 2017, an employee was wet grinding a piece of plate steel on the Okamoto LSG Wet Grinder. The employee was feeling the surface of the steel plate for finish quality and the grinder wheel touched the employees right ring finger, grinding the fingernail off and fracturing the employee's finger. The employee did not ensure the grinding wheel returned to its home position before inspecting the steel plate. The wet grinder was located in the tooling department.
