NarrativeEmployee reached into the press #78 and his left index finger was severed at the second joint. The press has a pop up release which is activated after the cycle, the employee placed his hand while the pop up release was in operation catching the employee's hand between the part and the top of the press.
The accident investigation below was opened on Monday, June 28th 2010 by OSHA representitives in Atlantic, IA which had occured previously at Mahle Engine Components Usa, Inc.. A Machine operator had been injured when the employee losed finger in grinder accident

NarrativeEmployee #1 was operating a sander grinder, and he had decided to make a scraper to clean out a dip tank. Employee #1 was free handing the metal steel on the di sc, when it moved, amputating his left thumb. Employee #1 was taken to the Hospi tal, and had surgery to trim the bone.
