NarrativeAt 4:15 p.m. on December 11, 2017, an employee was working for a firm that printed business forms. He was pulling electrical wiring through a ceiling. He was using a new 2.4-meter (8-foot) Wagner stepladder. The employee fell from ladder as he was pulling the wires. There were no witnesses to the incident. He sustained fractures to his first and fifth lumbar vertebrae and additional fractures to the left side of the L1 through L4 lumbar vertebrae region. He was hospitalized.

NarrativeAt 12:40 p.m. on November 30, 2017, a 24-year-old employee was working for a commercial printer. He was feeding paper through the blanket rollers on the firms press unit Number 3, a Goss Sunday 3000 printing press, while the press was running in inch mode. This was known as "re-webbing" the press. The employees hand got too close to the blanket rollers. The middle and ring fingers on the employees right hand were caught between the rollers and crushed. He was not hospitalized.

NarrativeAt 8:30 p.m. on November 6, 2017, and employee was loading paper into a flat bed cloth cutting machine for cutting. While lining up the stack, she inadvertently stepped on the machine's activation foot peddle and the clamp came down on her finger. The employee's finger was amputated, which she was hospitalized and received treatment.
